Hazard description	The detachable tissue box toy can come apart, exposing the small support brackets to young children. There is a risk of serious injury or death by choking if the child places the small parts in their mouth._x000D_ _x000D_ _x000D_ Two reports worldwide of the tissue box toy coming apart have been received. No incidents have occurred in Australia. No injuries have been reported.
Remedy	Remove the tissue box toy from the activity centre immediately and place out of reach of children.
